Independent Living Skills Trainer The Center for Disability Rights, Inc. (CDR) a not-for-profit, community-based advocacy and service organization for people with all types of disabilities is currently seeking an Independent Living Skills Trainer Job Summary: With general supervision the ILST will help those with disabilities to become more functional independent through functional based assessments and creating comprehensive treatment plans. Duties and Responsibilities: ? Assessing and assisting individuals diagnosed with a disability in a one-to-one clinical setting in the home and community. ? Independent Livings Skills Trainers develop tools and strategies to help participants in the Traumatic Brain Injury Waiver Program and Nursing Home Transition and Diversion Program to perform activities of daily living and instrumental activities of daily living independently. ? This clinical position assists participants to work toward accomplishing their goals from individualized plans developed collaboratively with skills trainers. ? Independent Livings Skills Trainers act as a liaison between individuals with participants in the community and other service providers for continuity and quality service provision. Required Knowledge and Skills Education and Experience: Only candidates with either (A) type or (B) type qualifications will be considered for this position: Person with one of the following: Registered Occupational Therapist Registered Physical Therapist Licensed Speech-Language Pathologist Registered Professional Nurse Certified Special Education Teacher Certified Rehabilitation Counselor Master of Social Work Master or Doctorate of Psychology Plus: Candidate must have minimum, one (1) years of experience completing functionally based assessments, developing a comprehensive treatment plan and teaching individuals with disabilities to be more functionally independent -OR- (B) Person with a bachelor?s degree in a human services related field And Minimum of two (2) years of experience completing functionally based assessments, developing a comprehensive treatment plan and teaching individuals with disabilities to be more functionally independent Note: Please clearly state all applicable qualifications on submitted resume Position Specifics: Location: Rochester, NY Salary: Shift: (must be flexible) 9am till 5pm Monday ?
