Job Description

Working under direct clinical supervision, a Social Worker embodies the healing commitment of providing extraordinary care to our patients.  A Social Worker utilizes clinical expertise to participate in the development and implementation of a plan of care that provides the best outcome for the patient.    This is a great opportunity to work collaboratively with patients, patients’ families, healthcare providers, insurers, and community resources.  The Social Worker may provide initial intake, crisis intervention and adjustment to illness counseling services as well as appropriate referrals to treatment and resources.  They may coordinate the discharge planning process from patient admission to departure, screen for and identify patient needs, develop a discharge plan, and coordinate with internal and external services for care during and after a hospital stay They provide services appropriate to assigned patient population as established with the plan of care and in accordance with professional licensure.  These services may take place at a hospital, facility, clinic, inpatient, outpatient or home care setting.

Entry Rate:  $16.79 (SSW), $20.25 (CSW), $22.13 (LCSW)  

Minimum Requirements

• Bachelor's degree or higher in Social Work.  Intermountain verifies both degree attainment and educational institution accreditation following an offer of employment.
• Current licensein the State of Utah as a Social Services Worker
• Excellent interpersonal skills, documentation skills, and must be able to work in an interdisciplinary health care setting
• Must be bilingual in English and Spanish

Preferred Qualifications

• Previous experience working as a Social Work Intern or Social Work experience in a medical setting
